VA 26-0286 Loan Summary

Use the VA 26-0286 Loan Summary form to provide basic information on each completed loan to the VA. The VA uses this information to establish internal loan records and to assist in issuing the Loan Guarantee Certificate.

  • At the top of the form, type the VA Agency Case No. This is the same as the VA Loan Number.

Veteran Information

  1. Select Borrower or Co-Borrower from the Veteran Information for dropdown list.

    • Borrower or co-borrower information is copied from the Borrower Summary and page 3 of the 1003 application.

  2. On lines 7 and 8, type the Entitlement Code and Entitlement Amount from the VA Certificate of Eligibility completed and returned by the Veterans Administration.

  3. On lines 9 and 10, select the Branch of Service and Status (current duty status).

Loan and Property Information

  1. Select options from the dropdown lists on lines 12 through 18 to specify the properties of the loan.

  2. For a refinance type loan, select checkboxes for line 22 to indicate any energy improvements and then type the Amount Spent on Energy Improvements.

  3. In lines 24 through 27 and 30, select options to describe the subject property.

  4. If you selected MCRV for the Appraisal Type on line 25, type the MCRV # on line 29.

  5. On lines 36 and 37, type the Lender and Agent IDs assigned by the Veteran's Administration.

  6. On line 38 type the number assigned to the loan by the lender.

For LAPP Cases Only

If you selected LAPP-Lender for the Appraisal Type on line 25, type the additional subject property information.

Income Information

  1. On lines 47a through 47c, select options from the dropdown lists to indicate whether the VA loan was underwritten by an automated underwriting system.

    • If you clicked Yes for line 47a, specify the automated system used in line 47b, and the resulting Risk Classification on line 47c.

    • If you clicked No for line 47a, leave lines 47b and 47c blank

    The Residual Income Guideline value on line 52 is the Guideline value from line 44 of the VA266393 Loan Analysis form.

  2. On line 54, select Yes or No from the dropdown list to indicate whether the spouse's income was considered.

Discount Information

Type discount points, loan terms, interest rate, and then indicate whether or not the borrower is exempt from the Funding Fee.

For IRRRLS Only

For an Interest Rate Reduction Refinancing Loan, type information related to the loan being refinanced.
